E.4.23.GL – Centerstone Training Manual Addendum

Site Learning Activities and Requirements #

All interns will participate in the internship learning activities as specified in the manual. Specific site activities/rotations/requirements above and beyond what is listed in the manual are provided below:

Interns will participate in treatment team meetings by accompanying the supervising psychologist. This provides interns with the ability to engage in assisting teams with staffing difficult cases while also observing the processes involved in providing integrated care in community mental health across multiple settings. Interns, along with the Training Director, will develop a learning plan in the beginning of the internship year in order to identify areas of interest and focus for training. While interns will gain experience across all service lines offered at Centerstone, they may choose to focus and spend more time in particular areas of interest as team members in a multidisciplinary model. Primary intervention modalities can include group and/or individual psychotherapy, as well as assessment opportunities. As a broad overview, the Columbus, Indiana Centerstone office provides services in the following team structures:

Crisis/Access: Conducts intake evaluations, assesses level of need, and coordinates internal referrals to appropriate services. Team members also provide emergency services to clients or community members in crisis, and may assist with arranging acute inpatient care in an appropriate external facility or admission to the least restrictive outpatient program. In addition, this team provides outpatient services for clients in need of brief psychotherapy intervention.

Child & Family: Offers services to children and adolescents, as well as their families. This team works closely with community organizations such as the local school district as well as the Department of Child Services. Interns will provide individual and family psychotherapy within the clinic or via telehealth. Team members include therapists, family support specialists, and care coordinators.

Adult & Family: Provides services to adults with severe and persistent mental health needs. Interns will provide outpatient psychotherapy within the office or via telehealth, while other team members provide community based interventions to clients with severe mental illnesses. Team members include therapists, rehab specialists, health coaches, and care coordinators.

Addictions Services: Within Adult & Family Services, the Addictions team members provide comprehensive Substance Use Disorder interventions. Interventions include individual psychotherapy, group-based interventions (including Motivation for Change, Intensive Outpatient, Aftercare, Seeking Safety, and Dual Diagnosis groups), and community based intervention provided by recovery coaches and peer recovery specialists. Team members also work closely with medical staff providing medication-assisted treatment.

Medical Services: Psychiatric services are provided on-site and medical providers work closely with staff members to provide comprehensive care to children, adolescents, and adults. Providers include psychiatrists and advanced nurse practitioners along with the support of nursing and medical assistants. Interns will work alongside medical staff on a regular basis, and have the opportunity to observe providers in session.

Integrated Health: Two traditional interns can elect to have a year long rotation in a corporate medical clinic (LiveWell Center) in Columbus, Indiana through a partnership between Centerstone and Cummins Engine Company. Interns will gain experience in integrated health, being placed in the clinic 2 days per week. Interns will provide short term treatment with a multinational client population referred by medical and ancillary staff. Interns will be integrated into the clinic, providing consultation to the medical staff on psychological issues impacting compliance with medical treatment, the stressors of coping with both acute and chronic physical health challenges a patient may be experiencing, and treating a broad range of diagnosis. In addition, interns will receive warm handoffs from medical staff to provide immediate psychological interventions, or provide crisis evaluations when needed. Interns will develop a diverse caseload of Cummins employees and their families.

In lieu of the integrated health placement, interns will be placed in a small county office for a year. This will provide the intern means to engage with rural and underserved populations and gain experience as a clinician in a rural mental health clinic. They will have the opportunity to provide a full range of services including both short and longer term individual, group, and family psychotherapy. Interns will also receive referrals for psychological assessment from clinical staff.

Integrated Health Care Internship: This internship differs from the traditional internship. Here, the intern will be placed full time in Centerstone’s new Federally Qualified Medical Clinic (Look-Alike) in Columbus, Indiana. The intern may also work one day a week in the Bloomington health clinic. The intern will be fully integrated into the medical clinic staff, providing psychological services to an underserved population of adults and children with a broad spectrum of physical and mental health needs. Referrals to the intern will be made by the medical and psychiatric providers. Treatment will be short term and solution focused. The intern will have the opportunity to work with clients who are experiencing medical difficulties impacting their mental health, or have psychological difficulties contributing to their resistance to follow medical recommendations. This might include, but not limited to, referrals for weight management, improved compliance with their diabetes regimen, or pain management. The intern will provide psychological consultation to the medical staff, receive warm handoffs to provide immediate care of patients, provide crisis assessment and intervention when needed and maintain a caseload of ongoing clients. This intern will be required to conduct appropriate psychological evaluations and attend team staffing with their Traditional Intern cohorts.

It is expected that the internship and training activities come first and any other roles/duties are secondary to the completion of the internship program.
