Site Learning Activities and Requirements

All interns will participate in the internship learning activities as specified in the manual. Specific site activities/rotations/requirements above and beyond what is listed in the manual are provided below:

CHCW offers a variety of experiences for interns related to behavioral health integration.  Throughout the year, interns will complete three 16-week rotations.  Below is an overview of the rotations and experiences. Note that 1 clinic = ½ day for a total of 10 clinics per week

Rotations:

    ll All rotations include 6-7 clinics of PCBH service delivery in which Interns will provide efficient (e.g., 20-30 minutes) behavioral consultation services to patients. Interns will be available for immediate, same day referrals (i.e., warm-handoffs) from primary care providers. They will also consult with the primary care team regarding behavioral health concerns.  By the end of the year, interns are expected to function as independent BHCs and see on average 4-5 patients per clinic and 1.5 patients per hour. 

o   Pediatric Rotation (Locations: CWFM, YP)

o   Rural Rotation (Locations: CWFM, NMC)

o   Mixed Rotation (Locations: CWFM, NMC, YP) 

Other Experiences:

·       Group (1 – 2 clinics per month: GRP): Interns will assist in multiple groups offered at our clinics (e.g. chronic pain, teen health, and wellness group). They may also have the opportunity to identify unmet needs within the clinics and implement new group curriculums that address these needs.

·     Experiential Learning and Group Supervision (1 clinic per week: EL): One afternoon per week (except the week of consortium didactics) will be used to provide interns with an in-depth, interactive learning experience in which they will practice skills and discuss topics relevant to PCBH and the practice of psychology. When topics are relevant, they will join the medical residents for their didactics during this time. Lastly, this time will also be used for group supervision in the form of case presentations and consultation. This will take place at CWFM.

·        Live supervision: Approximately one clinic per month, interns will be observed during their clinic time by one of the primary supervisors or postdoctoral residents. The purpose of these observations is to provide real-time feedback on the intern development, provide opportunity for supervisor to model skills, and to gather data to be used for the quarterly evaluations. The supervisor will use the observation tools provided in the forms section.

·        Projects and Paperwork Time (.5-1.5 clinics per week: PPW):Interns will have protected time each weekto complete paperwork and work on projects. Projects include the Critical Analysis (see NPTC manual), the self-directed journal review, and the Quality Improvement project (described below). A minimum of 2hrs, with most weeks having 4-6 hours, will be allotted for this. Provided that paperwork is complete and adequate progress is being made on projects, this time may also be used for dissertation or other scholarly activities of the interns choosing.

o   Quality Improvement Project:  Interns will design and implement a quality improvement project at a CHCW clinic. Quality improvement consists of systematic and continuous actions that lead to measurable improvement in healthcare settings and the health status of targeted patient groups. Interns will choose from a wide variety of topics/project needs and are encouraged to focus on a quality improvement area that interests them. Interns will meet monthly with the supervisor designated as the project adviser, for assistance in development and implementation of the project. The project will culminate in a presentation, forum TBA. Projects will not be penalized based on whether or not improvements are made but rather the thought, planning, and critical analysis that is put into the project.

It is expected that the internship and training activities come first and any other roles/duties are secondary to the completion of the internship program.

Administrative Overview

Salary:

$40,000

Schedule:

Full-time (40-45 hours per week)

Overtime:

Some overtime work is required, but interns should never work over 50 hours per week.

Nights/Weekends:

Clinics are open until 8:00pm five days a week and interns may be expected to work one late shift per week.

Malpractice Insurance:

If an intern is not covered by their school, CHCW will provide insurance coverage.

Mileage:

CHCW follows the NPTC Travel Policy with regard to intern travel reimbursement when didactic training is located at HealthPoint in Renton, WA.

Benefits:

Interns are eligible for benefits offered to other site employees. Benefits will be discussed in detail during site orientation.

Leave Time

Paid Time Off (PTO):

14 days (112 hours) combined vacation and sick leave

Professional Development Leave:

3 days (24 hours) of professional development leave represented as CME.Professional Development time is not considered PTO time. To use this time, interns will coordinate their schedules with their Primary Supervisor and Training Director, and track the hours as CME hours in their local system and as “Professional Development Leave” in the NPTC time tracking portal. 

Please Note: All 17 days cannot be taken within one rotation. In an effort to promote self-care, interns are encouraged to utilize all leave time by planning ahead for time-off throughout the year, as well as considering time needed for professional needs (e.g. dissertation defense, post-doc interviews) and sick time. Interns are encouraged to utilize all of their PTO and Professional Development/CME time before the end of the year, as CHCW does not pay out unused PTO and/or CME time. When a scheduled holiday falls on a weekend the observed holiday is assigned according to individual site policy.

Additional Information

Interns are expected to abide by CHCW policies, which will be covered during CHCW orientation. Interns will also be provided with a written copy of these policies. Below are policies specific to interns.

Time-Off Requests

Purpose: A great deal of planning goes into creating schedules that meet a variety of needs within CHCW. That includes patients’ needs for appointments, colleagues’ needs for support, and trainees’ needs for an enriching learning experience.  At CWFM, the master clinic schedule is completed and sent out more than 30 days in advance. When PTO is approved prior to the master schedule coming out, it can be reflected on the master schedule and adjustments can be made to maximize BHC availability and to minimize the impact on patients, colleagues, and the training experience. This also encourages trainees to plan out their PTO and be intentional about their wellness. 

Policy: The BH Program Coordinator sends an email to request PTO submission prior to completing the next schedule block, typically 60 days in advance. The BH Interns will make every effort to have their PTO approved per the request deadline. Additionally, all 17 days cannot be taken in one rotation. Requests are not considered approved until steps of procedure have been completed and approved by all parties.

Procedure for planned events:

·         PTO is only considered approved once intern has received “approved” responses from above supervisors and time is appropriately entered into the timesheet.

Call-Outs: From time-to-time unforeseen events, such as illness, will prevent interns from attending their scheduled activities. If these events occur with more than 24hr notice follow procedure above.

Chart Completion

Purpose: Timely documentation of patient encounters in the electronic health record is essential to quality patient care and safety. Other team members rely on these notes to be accurate and up-to-date regarding patient care. Timely completion of documentation is critical in meeting these needs.

Chart Completion: Charts must be completed and submitted for initial review to the intern’s individual supervisor within three business days, including PTO days. Edits requested by supervisor must be completed within one business day.  In the event that this does not occur, the intern will be taken out of clinic until charts are complete. Time out of clinic will be made up..

Checking and Responding to Email

Purpose: Pertinent and time-sensitive information is frequently communicated via email.

Email: Email needs to be checked each business day and responses, when required, sent within two business days.

Productivity Expectations

Purpose: The CHCW internship is designed to train psychology interns to be behavioral health consultants in the Primary Care Behavioral Health Model of care. A core component of this model is to operate within the fast-paced, highly productive, primary care setting. Over the course of the year, interns will learn and employ strategies to efficiently and effectively see a large number of patients. Interns will learn self-monitoring strategies and partner with supervisors to meet these goals.Expectations: Interns will be expected to complete, at minimum, the following number of patient visits per clinic, depending on where they are in their training year: 2-3 during 1st rotation; 3-3.5 during 2nd rotation; 4-5 during their 3rd rotation. At least once per week, interns will enter the number of patients and clinics into “Patients per clinic tracking” excel spreadsheet in the Psychology Interns and Fellow OneDrive folder. This will be reviewed at least monthly during supervision and incorporated in the NPTC evaluations.
